General Terms

Payment for therapy sessions are due in full at the session. Cash, personal checks, major credit cards and Health Savings Accounts are welcome. You may be eligible for reimbursement from your insurance company. See information below.

Our cancellation policy requires that you notify us of your need to cancel at least 24 hours in advance of your scheduled appointment time, or you will be charged the full amount of the missed appointment.

For a missed appointment, we reserve the right to charge your credit card on file. A receipt will be provided, however, it will not include diagnostic and treatment codes for missed appointments since no therapy services were provided. This means that charges for missed appointments cannot be submitted as a claim to an insurance provider for possible reimbursement since no therapy services were provided.  

 

Private Insurance

Essence Physical Therapy does not directly bill health insurance providers. Patients pay in full at the time of service and will receive a statement to submit themselves to their insurance provider for reimbursement.

Patients should contact their health insurance provider prior to the first visit to review coverage. The following questions are recommended.

  • What are my ‘out of network’ physical therapy and/or occupational therapy benefits?
  • What is my deductible and has it been met?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is a referral/ prescription required from my primary care physician?

Initially, you do not need a physician’s referral to see a physical therapist. However, your insurance provider may require a prescription for physical therapy services in order to process your claim. Please ask your insurance provider if a prescription is necessary prior to your first visit.
